The Pennsylvania Family Group Decision Making (FGDM) Statewide Evaluation captures information regarding FGDM practice statewide, including data about conference participants’ experiences, model fidelity, and child-level outcomes. The FGDM Evaluation Portal allows users to enter and view evaluation information specific to their county.


Counties participating in the FGDM Evaluation can complete and submit Baseline and Outcome forms online.

  • Complete this form 45-60 days post-conference
  • Complete this form again at 6 months post-conference

  • This report displays all Baselines and their corresponding Outcomes (if entered) since
    December 13th, 2017.



The FGDM Evaluation Dashboards present data for conferences held after July 1, 2015 in charts, graphs and tables to allow for easier interpretation of evaluation results. Users may also view and filter statewide and county data specific to their interests and needs.

The Pennsylvania FGDM Leadership Team remains committed to the PA FGDM Statewide Evaluation. The evaluation is advised by the FGDM Evaluation Committee and managed by the University of Pittsburgh Pennsylvania Child Welfare Resource Center (PACWRC). Participation in the evaluation is voluntary.
