The Pennsylvania Family Group Decision Making (FGDM) Statewide Evaluation captures information regarding FGDM practice statewide, including data about conference participants' experiences, model fidelity, and child-level outcomes. The FGDM Evaluation Portal allows users to enter and view evaluation information specific to their county.

The paper versions of all the evaluation forms are available on the FGDM Statewide Evaluation page for those who wish to continue using them. Surveys are currently only available in paper form and must be submitted by mail.
